Coupons are valid at any Lou Malnati’s location and are good for 6 months.

Please include payment with the slip in an envelope marked “Lou’s Fundraiser.”

Make checks payable to St. Elizabeth of the Trinity School.

Payment is due by Friday, October 25.

No printer to print out the  form? Just write down the information on a piece of paper, and send it in to the school office with payment.
