Religious Education Registration and Forms for 2019-2020
RELIGIOUS EDUCATION REGISTRATION INSTRUCTIONS
Welcome to our new simplified Religious Education Registration here at St. Tarcissus. Please complete all steps of the registration process. We ask that all parents of children in the RE program be familiar with all the rules, policies and procedures of the parish Religious Education Programs.
Step 1 – Family Handbook (Review the information)
Review the information about the Religious Education program here at St. Tarcissus. The information is on our 2019–2020 Family Handbook (pdf Document). Rules, policies and procedures are in the family handbook.
Step 2 – Past Due Tuition (Returning Families)
Families need to pay any past due RE tuition (from the prior year) or have made arrangements with the program direction or pastor before enrolling students for the new year.
Step 3 – New Student Intake Form (Only for New Students)
Each student new to the Religious Education Program needs a completed New Student Intake Form (pdf Document). We also need a photo copy of each new student’s Baptismal Certificate for any student not baptized at St. Tarcissus.
Step 4 – Family Registration Card (Family Enrollment Form)
Complete the Family Registration Card (pdf Document). Submit this form with full payment or one-half of the total tuition amount for the 2019-2020 School Year. This card is used to enroll all the students from a family into the RE Program and to calculate the tuition amount. Please note that this does not include Sacrament Fees, or Late Fees.
Step 5 – Parent Fair Share Program Form (Parent Volunteer Program Information)
Each family needs to complete a Parent Fair Share Form (pdf Document). Each family needs to assist with three RE events. Families may sign up for volunteer events by submitting the Parent Fair Share Form. We will also allow parents to sign up electronically using “Sign Up Genius.” Additional information on the online signup will be out in September.

Please note that RE Catechists (teachers) receive free tuition for a child and Catechist Aides receive 50% off tuition.
Step 6 – Flocknote Form (only for New Families or Changed Email Addresses)
We communicate with parents by email. We use a system called Flocknote to get important information to parents and their children. We ask that all families give us a main contact email. We ask that we get only two emails per family as we pay for each email on the system. Use this Flocknote Form (pdf Document).
Step 7 – Emergency Contact Form / Special Student Needs Form
Each family must have a completed Emergency Contact Form on file. It must list three different persons with phone numbers that we can contact in case of an emergency. One of the numbers should be for a person that does not live in your home, but that we can contact in an emergency. On the back of the card parents should list any special needs for your children (i.e. Joe has a allergy to peanuts). Use this Emergency Contact Form / Special Student Needs Form (pdf Document).
Step 8 – Optional Forms (Photo Opt Out and Choir Permission)
Photo Opt Out – We often take photos of students for use in the Parish Bulletin or Website. Sometimes photos are also used in printed materials. Names are not used with student photos. If you do not want your child photographed, you must fill out this “Photo Opt Out” Form (pdf Document).
Choir Permission – The RE Choir is open to students in grades 3 through 8. The Choir Permission Form gives the details of student and parent responsibilities (for choir members). The Choir Permission forms are available from the Choir Director.
